This Privacy Statement explains how Paradigm Health System, LLC, (“PHS”) protects and uses your information. This Statement is designed to keep you informed regarding your use of PHS’s services (collectively the “Services”) as well as what happens to the information you and other parties provide to PHS by using the Services. This Statement applies to all of the Services provided by PHS including the use of PHS’ website and other online platforms.
PHS reserves the right to modify this Statement at any time. We will notify you of any material changes to the Statement by positing the modified Statement at https://paradigmhealth.net/privacy and changing the Effective Date at the top of the Statement. This Statement is incorporated into PHS’ Terms and Conditions.

1. What information is collected?
We do not collect personally identifiable information except where you specifically provide us with such information. With your consent, we may collect your full name, company name, web address, telephone number, and other contact information. We may also collect credit card information from you. We generally collect this personal information on our registration and order forms when you sign up to receive the Services. Aggregate technical information (such as IP addresses, screen resolution, browser language, etc.) and anonymous demographic information (such as your postal or zip code, the website that you came from, etc.) is collected during your usage of the Services).
The Services that we provide are focused on the provision medical treatment and related services as well as your access to your important healthcare information through our patient portals. In the provision of this process certain data may be collected including information relating to your business practices including your customers, marketing behavior, clients, sales, revenue information, online traffic, and other important datasets. If you use the Services, you are providing us with this important and sensitive information. If you do not wish to provide this important sensitive information to us, do not use the Services.
2. How is the information collected?
You provide us with certain information when you sign up for the Services. Information will also be collected through interaction with end users including location information, IP addresses, and other data reflecting their interaction with PHS’ website, web portals, and health care providers. To the extent that this information constitutes information protected by The Health Insurance Portability and Accountability Act (HIPAA) of 1996, PHS refers you to the Terms and Conditions for a full explanation of your rights and PHS’ obligations for protection of that information under HIPAA.
3. What is done with the information collected?
We use your information, including personal information, in order to prevent fraudulent transactions, to prevent unauthorized access to your account, to improve the content of the Services in order to enhance your experience, to contact you when necessary, and if you have consented and you have not withdrawn your consent, to contact you with marketing communications in relation to the Services (see Direct Marketing below).
Whenever possible, we use non-identifiable information (such as IP addresses and anonymous demographic information) rather than personal information. Non-identifiable information is used to tailor your experiences with the Services by showing content in which we think you will be interested and displaying content according to your preferences. We may use aggregate data for a variety of purposes, including analyzing user behavior and characteristics in order to measure interest in (and us of) the various features of the Services. We also use the information collected to evaluate and improve the Services.
4. Sharing information with third parties.
We work with third party companies who help us provide and improve our Services or who use advertising or related products. Our work with these third parties helps enable to provide aspects of the Services at no cost to our Clients.
None of the information we share with third parties contains information that personally identifies you (such as your name, email address, and contact information). We may provide third parties with information relating to your use of the Services. By using the Services, you are giving us the consent to share your information with third parties. If you do not wish us to disclose this information to third parties, do not use the Services.
We may also disclose your information in special cases if required to do so by law, court order, or other governmental authority, or when we believe in good faith that disclosing this information is otherwise necessary or advisable, such as to identify, contact, or bring legal action against someone who may be causing injury to – or interfering with – the rights or property of PHS, the Services, another user, or anyone else that could be harmed by such activities (for example, identity theft or fraud).
5. Children’s Online Privacy Protection Act
PHS does not market to nor seek users under the age of 13. Information discovered to contain personally identifiable information on an individual under the age of thirteen will be immediately purged and never shared with third parties.
6. California Users
Clients who reside in California and have provided their personal information to us may request information about our disclosures of certain categories of personal information to third parties for their direct marketing purposes. Such requests must be submitted to us at contact@paradigmhealth.net. Within thirty days of receiving such a request, we will provide a list of the categories of personal information disclosed to third parties for third-party direct marketing purposes during the immediately preceding calendar year, along with the names and addresses of these third parties. This request may be made no more than once per calendar year. We reserve our right not to respond to requests submitted other than to the address specified in this paragraph.
Notwithstanding any other provision, we may also engage a data provider who may collect web log data from you (including IP address and information about your browser or operating system), or place or recognize a unique cookie on your browser to enable you to receive customized ads or content. These cookies contain no personally identifiable information. The cookies may reflect de-identified demographic or other data linked to data you voluntarily have submitted to us, e.g., your email address, that we may share with a data provider solely in hashed, non-human readable form. 7. Direct Marketing
With your consent, we and/or affiliate companies may sometimes contact you (by email, SMS text, letter or phone) in order to provide targeted marketing about our Services or their services (as the case may be). Such marketing communications will only be sent to you if you have given your consent when you registered for our Services and have not withdrawn such consent.
All marketing e-mails you receive from us or companies we work with will include specific instructions on how to unsubscribe and you may unsubscribe at any time.

8. What are cookies and how are they used by PHS?
A cookie is simply a tiny text file containing pieces of data, stored when you visit a website. It’s designed to help websites remember what you did in the past. This can include whether you clicked on particular links or pages or read pages on the site as long as the cookie is valid.

9. How long is information retained?
Different information is used for different purposes, and is subject to different standards and regulations. In general, information is retained for such period of time as is felt necessary to provide you with services you request, to comply with applicable regulations or legislation, and to ensure that you have a reasonable opportunity to access the information. 10. Are the Services secure?
PHS takes necessary precautions to preserve data security, based on the nature of your data and the risks posed by our processing, and in particular to prevent their being impaired, damaged or having unauthorized third parties get access to them (physical protection of premises, authentication procedures for our clients with personal and secure access using confidential identifiers and passwords, logging of connections, encryption of certain data, etc.).
